Putnam County Schools - Pre-Kindergarten Registration

KINDERGARTEN REGISTRATION 2008
for the 2008-2009 School Year

APRIL 10, 2008
1:00 pm - 5:00 pm

Child must be 5 years of age by September 30th
Transportation districts can be confirmed by calling School Transportation at 526-9342.
Please bring the following documents with you when you register your child for kindergarten:
- Birth Certificate
- Tennessee Certification of Immunization
- Physical Examination (conducted during the 2008 calendar year)
- Child’s Social Security Card
- Two (2) Proofs of Putnam County residency: utility bill, tax statement, voter registration card, notorized letter from rentor.
- Proof of custody if the child does not live with one or both natural parents

For Your Information…

Tennessee has mandated enrollment caps for all K-4 classes. It is very important to register for the school your child will be attending .
